ARE YOU A CARER?

Do you care for a family member or a friend on a voluntary basis? Text CARER to 87007 for free confidential information, advice and support from the Wirral Carers Health and Wellbeing Service.

Be sure to inform a member of Sunlight Group Practice that you are a carer so that you can be added to our carers register.

A carer is anyone, including children and adults who looks after a family member, partner or friend who needs help because of their illness, frailty, disability, a mental health problem or an addiction and cannot cope without their support. The care they give is unpaid.

It is important that you access any help or support you need, to ensure your own wellbeing as well as that of the person you care for.

Young Carers – Barnardo’s

Barnardo’s support children and young people who care for and look after family members affected by health issues, disability or substance misuse. Barnardo’s help them look after their loved ones and give them time to enjoy their childhoods.

The website has information about carers support groups, events, access to services as well as information to support independent living for those that you care for.

Herbert Protocol (Dementia)

People with dementia can sometimes start to wander. This might only be into the garden or street for a short time, but sometimes people get lost and go missing.

You can fill in this form on your computer or print it out and fill it in by hand:

  • Keep it somewhere safe where you can easily find it if the person goes missing.
  • Give a copy to friends, family and neighbours.
  • Keep this information up to date whenever something changes.
  • They only need to give the form to the police if the person goes missing.
  • Details are helpful, but tell them not to worry if they can’t answer every question.
  • There is space at the end of the form to tell us more if they run out of space for any of the questions
  • They can always add another page to a printout or add something to an email.

Keeping a completed form means you don’t have to try to remember the information when you are under stress if someone goes missing. And it saves time, so we can start the search sooner.
