Register with our practice

If you are within our Practice boundary, we are happy for you to register with us. To check you are within our boundary, enter your postcode below.

  • Online – You can register with the practice online, this is a much quicker way to register and provides us with more information so your patient record is more accurate. You can now register online using the link below.
  • In-person – You, or someone on your behalf, can fill in a registration form and practice health questionnaire at one of our practice locations.

Once you have registered with us, you will be contacted by the practice for a patient health check.

Finding your NHS number

To make registering as straightforward as possible, it helps to have you NHS number, this can be found by either visiting the NHS number finder page or contacting your current GP surgery and asking them.

Catchment Area (Practice Boundary)

If the pin does not appear within the practice boundary, you are unfortunately not within the catchment area.

Named (accountable) GP

All practices are required to provide their patients (including children) with a named GP who will have overall responsibility for the care and support that the surgery provides to them.

At Sunlight Group Practice all patients are registered to our one of our Partners, they are your accountable GP. You may of course see any GP in the practice as you currently do and normal booking procedures still apply. Your named GP will be stated on your prescription, or available upon request. If you want to know who your named GP is, please feel free to contact us.

Accessing someone else's information

As a parent, family member or carer, you may need to add someone to your NHS App. This is sometimes called having a linked profile or having proxy access.

It needs to be set up by a GP surgery where you and the other person are both registered.

Proxy access was developed to allow someone other than the patient to access and manage parts of their GP online services account.

The proxy is given their own online access account (rather than using the patient’s login details). It is often used by parents or recognised carers of young children, and recognised carers of adults, using the NHS App or by logging in through the NHS website.

Setting up access

You and the other person need to be patients at the same surgery.

You’ll need to provide identification and the person will need to consent, when possible. This is so we can confirm who you are and check it’s appropriate for you to act for the other person.

Depending on the level of access your surgery agrees to give you, you may be able to:

  • Request and manage their repeat prescriptions
  • View some or all of their GP health record
  • Book an appointment for the other person
